FPC evidence record: field-by-field reference
An FPC evidence record is useful when it lets a reviewer trace a supplied item or completed job back to the controlled process and source evidence. This reference explains the information teams commonly link together; it is not a prescribed form or a substitute for the applicable execution specification, project requirements or conformity assessment.

How to use this reference
This is a record-keeping reference to the fields that commonly appear on these documents, checked 26 September 2026 against the current published editions named on the page. It paraphrases field headings; it does not reproduce the standard, and it is not a substitute for it. Qualification ranges, validity rules and acceptance criteria are set by the standard, your code of construction and your certification arrangements, so read those before relying on any field here. Keep the traceability chain explicit
A useful chain is order and drawing revision → item or job → material heat → weld and WPS revision → welder qualification → inspection evidence → review and release. FPC is broader than a welding register; use your own FPC procedures and assessment scope to decide what evidence is required. See also the calibration record field reference.
